Thank you for your interest in the Behavior Place!

Five important documents that must be submitted to complete the intake process:  

• A prescription for ABA with no recommended hours; (Medical necessity is determined by the Licensed Behavior Analyst and approved by your insurance company);

• a diagnosis of an autism spectrum disorder (F84.0) from the doctor;

• a comprehensive evaluation from the doctor; Please make sure that there are observations, test scores run by the doctor such as ADOS, ADI and CARS

• a copy of the front and back of your insurance card.

• Most recent physical and immunization record
